Naples Heart Transplant Tragedy: The ‘Burned Heart’ Case of Domenico Unveiled

The tragic death of Domenico, a two-year-old boy in Naples, following a heart transplant has cast a dark shadow over the medical community, raising serious questions about alleged procedural errors and a ‘burned heart.’ Domenico, suffering from dilated cardiomyopathy, urgently needed a heart transplant. In December 2025, his family received the hopeful news of a compatible organ, but this hope quickly turned into a profound tragedy.

The Journey of a ‘Burned Heart’: From Bolzano to Naples

On December 22, 2025, Domenico’s family was informed of a compatible heart. The organ, sourced from Bolzano, reportedly arrived in Naples damaged. Investigations reveal a critical timeline:

  • December 23, 10:00 AM: The heart was explanted from the donor in Bolzano.
  • Allegations suggest the Naples medical team departed without an approved transport box, crucial for maintaining a controlled temperature.
  • Initial reports indicate Bolzano medical staff used dry ice instead of natural ice for transport.
  • The heart, allegedly transported without due precautions, arrived in Naples damaged, exposed to temperatures as low as -78 degrees Celsius instead of the required 4 degrees.
  • December 23, 2:30 PM: The explant of Domenico’s diseased heart began. Crucially, doctors reportedly started this operation before verifying the health of the new organ. By the time the damage was discovered, it was too late to reverse the procedure.

Mother’s Plea: “My Son Continues to Fight”

Patrizia Mercolino, Domenico’s mother, shared her anguish and unwavering determination in an interview with ‘È sempre Cartabianca.’ “My son continues to fight. I always hope a solution will be found. He lived like any other child. He had therapy, but I tried to give him a normal life. His will to live comes entirely from him.”

Despite the devastating outcome, Patrizia expressed continued trust in the doctors. “My relationship with the doctors remains the same. I have never doubted the professionalism of the surgeon who operated on my son, but trust is missing. We will see in time.”

The Experts’ Verdict: Unsuitable for a Second Transplant

The possibility of a second transplant for Domenico was a beacon of hope. On February 17, a compatible heart was confirmed. However, a crucial evaluation on February 18 by a panel of experts from leading Italian centers at Monaldi Hospital delivered a devastating blow: Domenico was deemed unsuitable for another transplant.

The unanimous decision cited a general strain on other organs, particularly the kidneys and liver, which compromised the success of any new intervention. Furthermore, suspected nosocomial infections were believed to render the child ineligible, regardless of organ availability. During the evaluation, Domenico was briefly awakened from sedation to allow specialists to assess his neurological condition before the final verdict.

The Legal Battle Begins: “Time for Responsibility”

With the confirmation of inoperability, the family’s lawyer declared live on ‘Dentro la Notizia’ that “the time for hope is over, and the time for responsibility begins.” The defense has requested all medical documentation, including CT scans from February, to determine if the situation was compromised earlier and if alternative treatments, such as an artificial heart, could have been pursued 50 days prior.

Parallel to this, the Public Prosecutor’s investigation is focusing on the transport of the first heart from Bolzano. It is hypothesized that personnel were inadequately trained in using the latest generation container, and that dry ice caused the organ to freeze at -78 degrees Celsius.

The Irreversible Prognosis and Shared Care Plan

Tragically, after sedation was suspended for clinical assessments, Domenico did not regain consciousness. This led the medical team to define the prognosis as “certainly and without a shadow of a doubt unfavorable,” as reported by lawyer Petruzzi on ‘Dritto e Rovescio.’

Acknowledging the irreversibility of the situation, lawyer Francesco Petruzzi, acting on behalf of Domenico’s parents, formally requested the activation of the Shared Care Plan (PCC) at Monaldi Hospital. This legal framework, introduced in 2017, aims to prevent futile medical treatment. Petruzzi emphasized that this choice is not euthanasia but a shift in clinical strategy from seeking a cure, which is no longer possible, to solely alleviating suffering.

Domenico’s Passing and a Legacy of Hope

Domenico passed away at 9:20 AM on Saturday, February 21. The hospital confirmed his death, expressing profound sorrow and solidarity with the family. “The Strategic Management, along with all healthcare professionals, expresses its deepest condolences and joins with respect and moved participation with the family in this moment of immense pain.”

In the wake of their devastating loss, Domenico’s family, alongside lawyer Francesco Petruzzi, announced their intention to establish a foundation to support children awaiting transplants.

A City Mourns: Domenico’s Farewell

On Wednesday, March 4, Domenico’s funeral took place in Nola, with thousands gathering in Piazza Duomo for his final farewell. Images broadcast on ‘Dentro la Notizia’ showed his white coffin met with applause, white flowers, and dozens of balloons released into the sky. Many attendees wore t-shirts bearing Domenico’s face. High-ranking state officials, including Giorgia Meloni and the President of the Campania Region, also attended the service.

Domenico’s mother, Patrizia Mercolino, spoke movingly: “Today, if all these people have come, it is only thanks to Domenico. To his smile, his big eyes, his sweetness. At this moment, he is embracing us all. I hope this is not the last day we think of Domenico, but that each of us keeps him in a corner of our hearts. I love you, my son.”
